Cape Cod Healthcare Seeks
Community Benefits Grant Applicants
(Hyannis, MA – July 17, 2025) Cape Cod Healthcare (CCHC) is pleased to release its 2026 Annual Strategic Grant Request for Proposals (RFP). The 2026 RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County. Organizations are invited to submit proposals for programs or projects that improve residents’ physical and/or behavioral health.
The CCHC Community Benefits grants program is open to all nonprofit organizations operating in Barnstable County with programs that align with CCHC’s Community Benefits priorities.
Successful proposals will:
- Explain how the proposed program improves health
- Address one or more social determinants of health
- Promote and address health equity
- Describe how the project impacts residents of Barnstable County
Community Benefits will host two grant information sessions:
- Tuesday, August 5, at 1 p.m. In person at the Cape Cod Healthcare Foundation, 4 Bayview St., West Yarmouth
- Wednesday, August 6, 10 a.m., Virtual via Microsoft Teams; link to be provided upon registration
Cape Cod Healthcare Community Benefits will award up to $300,000 total in FY26 Annual Strategic Grants. Grants are for one year only and the maximum grant award is $30,000. The deadline for submission of strategic grant proposals is Sunday, September 14, 2025. Applicants will be notified of the awards in November.
